  • Community Service Time Sheet – for recording your community service hours for the Diversion program or any other community service requirements
  • Community Service Opportunities – a list of places in Athens that offer opportunities to do community service
  • Agreement for Legal Services– if you see an attorney at our office, you will be asked to read and sign this form. If you are having a phone appointment or have not read and signed this form for any reason before receiving legal services, please print, read and fill out the required areas. You can scan and email or fax it to us
  • Re-enrollment form – during the first three weeks of the quarter, you can re-enroll to receive our services. For Winter Quarter 2012, you can re-enroll to use our services until JANUARY 24th, 2012. See our fee section for details.
  • Driving Privilege Form – this form allows clients with license suspensions to request driving privileges for driving a vehicle for purposes that are necessary for employment or other important matters (doctor’s visits, groceries, commuting to school, etc). There is a $25 fee to apply for driving privileges and you must attach a copy of valid vehicle insurance. The forms are submitted to Athens County Municipal Court.
  • Expungement and Sealing Forms — these forms allow you to petition the court to seal a dismissal and expunge a criminal conviction. See the expunging or sealing your criminal record section for more information.
